Each of Ramon’s family members gave justification to the concepts of Palliative care in general. Although there were some concepts of palliative care that were not given justification because Ramon doesn’t have his own nurse to do the Nursing care plan. Nursing care plan includes assessment; subjective and objective, nursing diagnosis, planning, interventions and evaluation. In Ramon’s case, assessment should have been done regularly. Subjective data includes information directly from the patient’s feelings while objective datas are those measured and observed by the physicians. After initial assesment, a problem list should be made consisting possible reasons that affects the patient’s well being qlike family problems, medical diagnosis and many others. Diagnosis comes when the collected datas relates to a certain illness or disease. In planning of interventions to be used, the preparation should be specific, attainable, measureable, realistic and time-bound. Intervension are the methods to be used in order to meet the goals in planning. It includes what medications to be applied, possible surgeries, etc. Every intervension has a rationale like why a nursing action should be done and what is its basis. And lastly, evaluation are written reports about the improvement of patient or the contrary. In evaluation it is stated whether the goals in planing is met, partially met, or not met (By Jane Urie). As shown in the movie, Ramon didn’t want to be loved because he believes that a person undergoing his illness would be creating misery to the person that loves him. This implies that Ramon is having psychological and emotional distress and should have been assessed by a physician to be able to bring him back to life. However, in Julia’s case it was shown that she had undergone observations and treatments when she developed a disease called Cadasil due to her series of strokes, therefore palliative care was well addressed. Pain control for instance was well established in the movie because there was a scene when Ramon was given tranquilizer to stabilize him. Today, euthanasia has been legalized. It was being applied in cases of hopelessness in the recovery of the patient. â€Å"Eu† means good and â€Å"thanator† means death, euthanasia therefore means good death or mercy killing. There are different classifications euthanasia: voluntary, non-voluntary, involuntary, assisted, euthanasia by action, and euthanasia by omission. In voluntary, the patient was the one who requested his death. In non-voluntary, the person who has been killed made no request and gave no consent. In involuntary, the patientmade an expressed wish to the contrary. Assisted Euthanasia is perform with the assistance of the physician or the relatives of the patient. Euthanasia by action makes use of lethal injection while euthanasia by omission is the process wherein the patient is no longer being provided with his needs like food, water, medication, etc. The reasons why it is being practiced are to diminish unbearable pain, the right to commit suicide, and the belief that people should not be forced to stay alive (â€Å"Voluntary Euthanasia † Apr 18, 1996). Ramon wanted to die. Ramon believed that dying is an unevitable part of a man’s life and it is just normal to die. Due to his own beliefs Ramon passed a petition about legalizing suicide or the right to die. A paraplegic priest visited him to discourage his intention of suicide. His brother, Jose, also agrees with the priest and the church’s belief that suicide is immoral, that only God has the right to decide whether an individual should live or die. They believe that God has His reasons and purpose why a person still lives. It was Him wo gave you life, so it is also his decision to take it back. Thereof, this situation talks about morality (â€Å"The Sea Inside,†). As for the health providers, it also brings a great controversy whether to practice   Euthanasia or not. Health providers were trained to cure not to kill. And assisting a suicide contradicts the profession with the duty of â€Å"do no harm†. From the movie itself, cultural considerations and individual considerations was well established. There was a clear observation that the people around Ramon did not want to consider his attempt of killing himself. Ramon as an individual had also been given consideration; although his beliefs of death was unjust they still did not oppose to him because they considered his own perceptions and did not want to oppress his rights as an individual. For a nurse, I would consider his beliefs because it was his right but I would not help him accomplish his death. The tragic part of the story is that his family was well oriented and they have accepted that death is coming to Ramon. There was a scene in the movie wherein his father said â€Å"what’s worse having your son die on you, is the fact that he wanted to†. It only shows that he had accepted the death of his child but he had not accepted the fact that his son wanted to die. Ramon’s family had actually given him quality care and unconditional love. Although these were really not enough especially in the case of Ramon. From a nurse’s point of view, the care that the family could have been improved if there is a plan of care that was established. Ramon had supposedly had a life that is worth living. His family loved him and took care of him, he had a book published, two women were linked to him and the sympathy of the nation was on him but he refused all these and exchanged it to death. He had exchanged all the beautiful things he had in life because he justified his life as his. For years Ramon had actually internalized that death is in his hands and that he had the right to end his life. Generally speaking, Ramon had actually lived a life that is not worth living because he restraints himself from having so. The US foreign policy from 1945-1991 - Case Study Example This research will begin with the statement that the term Cold War was coined by Pulitzer prize-winning publicist Herbert Bayard Swope and refers to the intense feelings of hostility and the profound international tension and struggle for power between the the USA and then the USSR, both of which headed formidable alliances i.e. Allies and Russian satellites, respectively.   It started in 1945 and ended in 1989.   It was ‘cold’ because the relations between the two superpowers were icy but never came to a flashpoint verging on a shooting war.   The conflict involved political rivalry and advantage as well as an upper hand in the balance of power. The conflict was replete with clashes of competing ideologies i.e. between the democratic capitalist system of America and its allies and the socialist/communist system of the USSR and the satellite nations comprising the nations of the Warsaw Pact. The rivalry consisted of propaganda, military alliances, atomic arms devel opment, reconstruction programs and the rivalry to win the hearts and minds of the neutral countries, most especially the third world countries which might provide military bases, natural resources, and markets. As early as 1929, the USA and the USSR had kept each other at bay and at arms' length despite differences in political ideologies. This estrangement was intensified by the USA's policy of isolationism in the 1930's which muted whatever feelings of mistrust they had for each other. But relations were improved when the USA and the Soviet Union unexpectedly found themselves fighting side by side against fascist Germany in World War II. The warm relations, however, rapidly dissolved when midway through the war, the USA realized that the USSR was determined to retrieve all the territories in Eastern Europe that it lost prior to World War I and these are eastern Poland, Latvia, Lithuania, Estonia and parts of Romania and Finland. It became clear to USA that Stalin in his paranoid fear of Germany and its military might want all of Eastern Europe to serve as its buffer states and shields from German aggression. President Roosevelt stressed that USA as adherent of the principle of self-determination wanted these states to choose for themselves the kind of government they want. The other western allies looked at Poland as strategically the bulwark of Europe which when falling into the hands of the Soviets would open the floodgates of Russians invading Western Europe. The US fears were vindicated during the 1943 Teheran Conference when Stalin finally laid his cards on the table: that he expected territorial concessions in the form of the states of Eastern Europe. Justice, to Stalin demanded that Russia be recompensed from the deaths of 16 million Russians and the massive destruction and damage to Soviet properties and machinery during the first three years of war. It dawned on President Roosevelt that reality, tradition and history dictated that self-determinatio n among the eastern European states would be anathema to Russia as any freely elected government in Eastern Europe would be hostile to Soviet ideology. Both the Yalta Conference and the Potsdam Conference in 1945 further strained the relations almost to the breaking point as Russia treacherously set up a communist government in Lublin, Poland followed by its undermining of elected non-communist governments in Czechoslovakia and Hungary. It was inevitable that the Cold War ensued in early 1946 after Berlin and Germany were divided into 4 factions i.e. Russian, American, British and French.

The Dangers of Genetically Modified Foods Essay Despite the recent surge in media coverage regarding genetically modified foods (GM foods) many Americans are unaware of what these foods are, or the possible benefits and dangers associated with them. Sometimes referred to as agricultural biotechnology; genetic modifying means that the DNA of food has been altered in some way at a cellular level. Scientists have learned that by inserting the genetic information from one organism into another they can create food crops that grow larger, stay fresh longer and are equipped with their own pesticides. GM foods have been a common part of the national diet since the early 90s. As with many scientific advances; e. g. , using chemo therapy to treat cancer, or underwater oil drilling, there have been unexpected, negative side effects. Genetically modified foods are not the super-food they claim to be. Until further research has been done to verify their safety, and prove they do not pose a threat to human health, the environment, and agricultural bio-diversity the use of GM foods should be banned or drastically limited in the United States. Human Health Many opponents to GM foods have raised concerns regarding the vast potential for these crops to cause allergic reactions in humans. A medical investigation done in India in 2005 determined that hundreds of agricultural workers developed allergies after being exposed to genetically modified cotton that contained Bacillus thuringensis (bt) toxins (Saunders, Ho, Cummins, 2007). Bt toxins are common biological pesticides that can be found in products like Roundup; a popular brand of household weed killer. Monsanto, the makers of Roundup, is also currently the world leader in GM food production. Similar research in parts of China and Africa also found a significant increase in the number of allergic reactions reported by agricultural works (Saunders, Ho, Cummins, 2007). The Journal of Hematology Thromboembolic Diseases published the results of study done on rodents that yielded frightening results. It was determined that there was a significant detrimental effect to the liver and kidneys of rats fed a strictly GM maize diet. The negative effects were caused by the mutagenic properties of genetically altered foods and the presence of Bt toxins transferred to and detected in the blood of the animals. The study concluded that before these foods can be considered safe for human consumption there will need to be longer studies completed on the effects of the toxins (Mezzomo, Miranda-Vilela, Freire, Barbosa, Portiho, 2013). Bt toxins, which have also been found in crops of soy and corn, are added to crops in hopes of reducing pests like weeds without the use of external combatants. The Environment The toxins produced by GM crops are not only dangerous to humans; they can be harmful to the environment as well. One pledge listed on Monsanto’s website states â€Å"We will use sound and innovative science and thoughtful and effective stewardship to deliver high-quality products that are beneficial to our customers and to the environment† (Monsanto, 2013). Many people have questioned the truthfulness of this promise. Prior to the wide-spread use of GM crops, there were multiple concerns regarding the over-use of chemical pesticides applied through sprays. Consumers have been told that by genetically altering the foods so they can produce their own pesticides there would be a vast reduction in the need for sprayed chemicals. Recent studies have shown that this is not the case. A 2007 survey of farmers who grew maize, soy, and other pesticide tolerant crops showed an increase of 383 million pounds more pesticide used than they would have required with non-tolerant seeds. The use of herbicides rose 31. 4% from 2007 to 2008 on GM crops. It was shown that as the weeds become increasingly tolerant to common pesticides like Bt toxins there will be increased need for external chemical pesticides (Budinger, 2013). Washington State University researcher, Charles Benbrook, likened the overuse Bt toxins to the overuse of antibiotics to treat common infections and stated that Its very much the same dynamic in play. By genetically altering corn, soybeans, and cotton so they can be sprayed throughout the growing season, farmers are now spraying their fields three times a season. Naturally as the weeds grow more tolerant they pass that trait to next generation, creating what is commonly referred to as a â€Å"super-weed† (Koebler, 2013). Roundup is a glyphosate-based herbicide used in many GM crops. The makers of Roundup have long reported that this is a safe alternative to chemical pesticides. Although this type of herbicide may be more desirable than its purely chemical rivals there are still environmental factors that must be considered. The United States Department of Agriculture (USDA) has stated that glyphosate has been proven to be deadly to earthworms. The decreased number of earthworms has resulted in a significant decrease of the bird populations in many areas where GM crops are grown (USDA, 2013). There has been no ruling against the use of Bt toxins in the United States however the French court has over-ruled Monsanto’s claims that there are no harmful effects to humans or the environment in 2009 (Marshall, 2013). Threats to Agricultural Biodiversity Concerns have also been raised by scientists and environmentalists regarding unintended effects of Bt toxins on non-targeted animals and insects. If the weeds can grow tolerant of the pesticides, it is logical to believe that insect pests may develop a resistance as well. This will cause there to be a spike in the pest insect population causing and imbalance in their relationship to preying animals (John Innes Centre, 2013). The most alarming effects of pesticides are related to the recent phenomenon of disappearing bees known as Colony Collapse Disorder (CCD). This, however, is not related to Bt toxins; it is believed to be caused by neonicotinoids, a class of neuro-active insecticides produced by Bayer, the second world leader in GM crop production. A study done by The Harvard School of Public Health was able to recreate CCD by introducing neonicotinoids to four bee colonies. The study showed that bees exposed to high amount of neonicotinoids perished quickly, however the colonies exposed to small amounts over a period of four months also eventually succumbed to CCD. Alex Lu, an associate professor of environmental exposure biology in the Department of Environmental Health led the study and stated â€Å"â€Å"The significance of bees to agriculture cannot be underestimated, and it apparently doesn’t take much of the pesticide to affect the bees. Our experiment included pesticide amounts below what is normally present in the environment† (Chensheng Lu, Kenneth M. Warchol, Richard A. Callahan. 2012). False Promises It is arguable that GM food manufacturers could not anticipate such harmful effects on humans and the environment and therefore we can view their statements regarding health and safety as non-malicious, however, there have also been false claims made by Monsanto that make it extremely difficult for the consumers to have faith in their practices. A Monsanto-trained scientist, Dr. Florence Wambugu, claimed that the sweet potatoes grown on a test farm in Africa showed a higher yield that of the non-modified variety grown on the same farm. This caused many proponents of GM foods to claim that international cultivation of these crops will solve our world hunger problems by creating a sustainable food source. It was later discovered that the claims made by Wambugu were untrue and, in fact, the GM sweet potatoes died out much sooner than the naturally grown counter-part. A similar study done in Uganda has also shown that non-GM sweet potatoes produce higher yield. Multiple unsuccessful studies were done on gm sweet potatoes spanning a 12-year period and costing over $6 million (The Department of Nutrition, Food Science, and Packaging at San Jose State University, 2013). The World Bank and four United Nations agencies completed a study in 2008 and concluded that GM crops are not the solution for creating a sustainable food source. The study found that the sustainability of these crops is unreliable and whereas the yield was higher in some areas, it was significantly lower in others. The study also showed that organic and near organic farming had a much higher success rate in Africa where there is an emergent need for more food. (Vazakas, Arvanitoyannis, Baltas, 2007) There has been a limited amount of research done to prove the efficacy and safety of GM foods and despite claims that they are not safe for human consumption, they are becoming increasingly harder for Americans to avoid. Since the introduction of GM food there has been a substantial increase in the number of food allergies reported each year in the United States (Will Genetically Engineered Foods Cause Allergic Reactions?, 2006) . The World Health Organization has concluded that further research and monitoring of public health records will need to done before a conclusive answer can be reached (World Health Organization, 2013). It is imperative that the American people educate themselves on the negative side effects of GM foods and put a halt to the practices of GM food giants like Monsanto and Bayer. Gorbachev and the Fall of Soviet Communism Essay From the failure of Khrushchev’s Virgin Lands program, to Brezhnev’s economic stagnation, to the final dissolution of the Soviet system, the organisation of central planning went though a slow death agony. When the former Minister of Agriculture, Mikhail Gorbachev, took over in 1985, the system seemed to have a certain spark left in it, though this was to prove to be illusory. Gorbachev announced that the Soviet state will not bail out failing enterprises, some limited market reform will be instituted and that limited press freedoms will be countenanced. Gorbachev did not satisfy anyone, including US policymakers. Liberals such as Boris Yeltsin thought reforms needed to go farther, while hardliners like Boris Pugo thought Gorbachev was selling out the Great Experiment. While Gorbachev cannot be held responsible for the dissolution of the USSR, he did unleash forces that eventually led to its demise. First, admitting defeat in Afghanistan, he shook the confidence in Soviet military might, unleashing a storm of protest from the â€Å"mothers movement,† embarrassing the USSR worldwide. Second, by letting up on police pressure, the huge black marketeers came to the surface, with money and local influence. Third, after Chernobyl, Ukrainian nationalism found new life in the RUKH movement, supported by a strong and large Ukrainian diaspora in America and Canada. Fourth, with the USSR clearly weakening, Islamic powers began to proselytize in the southern part of the USSR, such as in Azerbaijan and throughout the Caucuses. Fifth, the Baltics, sensing a weakening USSR, began to agitate for independence with substantial western–notably Swedish–help. Sixth, by retaining a hard line with Ronald Regan at the Iceland conference, the US began construction of a missile shield that forced the USSR to eventually backpedal on previous bellicose statements. Seventh, Gorbachev made it clear, just as he was not going to bail out failing enterprises, he also was not going to bail out failed regimes in Europe, starting with Ernst Honecker’s East Germany . Eighth, by proclaiming some sort of limited market, foreign goods rushed into the country, and new local elites began to form. Ninth, the reign of glasnost’, or openness, permitted activists more and more access to Soviet files, proving corruption and mismanagement. The fall of the USSR, based on the above events (and much more), had substantial implications for European politics. Firstly, the Swedes sought, successfully, to make economic colonies out of the Baltic states, especially Estonia. Their independence meant cheap and educated labor, and a new market for Swedish goods. Secondly, Germany was to be unified peacefully and without substantial protest. Again, this meant for Germany another economic colony of both cheap and educated labor, with a strong technical labor force. German investment eastward irritated France, seeing Germany ideally placed to take advantage of new Slavic investments in Poland, Russia and the newly formed Czech Republic (Czechia in Europe). France, as a result, sought to enlarge both NATO and the European Community (at the time) by bringing in new eastern states, including Turkey so as to counterbalance German expansion. The American response was typical. George Bush waffled on major issues, providing statements both for and against the independence of the Baltics and Ukraine. War flared up between newly independent Armena and Azerbaijan, as Islamic fundamentalists and drugs from Central Asia flooded the Caucuses on their way to markets in Europe and America,. Yugoslavia, a short time, later broke up along the same lines as the USSR under the incompetent leadership of Slobodan Milosevic, whose inability to control inflation led to the final destruction of that experimental state. US policymakers realized that they needed to move fast, while the USSR was prostrate, to begin monopolizing oil and gas resources throughout the country. The CIA began to step up its efforts in the Caucuses, making sure pro-American government were installed and properly trained. George Soros was a strong supporter of this move, and became a major political player in the region. In the mind of the US elites of both major parties, the fall of the USSR meant a green light for unlimited intervention in the affairs of former Soviet clients such as Iraq and Syria, and the Soviet backed regimes of Angola and Ethiopia were themselves were soon to fall to pro-US forces. Because of this, the latter nation fell apart into, like Yugoslavia, its original component parts. The New Left in the US, France and England scrambled to find another raison d’etre, and found it in diversity and multiculturalism. Great fear developed in the west when it became clear that Soviet (nuclear) scientists were not being paid, and were leasing out their considerable abilities to regimes opposed to US global rule such as Iran and Algeria. While all of this was happening, the US, and Harvard University, working with the World Bank, began implementing â€Å"shock therapy† to the liberated nations of eastern Europe. The rapid privatization in nearly all former Soviet satellites (except Belarus, who has maintained a high standard of living precisely by ignoring such moves ), led to mass impoverishment, rigged auctions, the rise of a nearly all-powerful mafia (with strong ties to Israel, and hence, to the US), who then assisted in the development of Russian â€Å"political parties. † Billions of dollars left the former USSR and its satellites, finding its was to Swiss banks and elsewhere, as wealthy organized crimes figures such as Symon Mogilevic began buying heavily into western economic life. There seems to be two distinct forms of former satellites. Those with direct and motivated patrons had an easy time of adjustment, as they became a part of western economic life quickly. The Baltics (Sweden), Slovenia (Germany, from Yugoslavia) and, clearly, East Germany, fall into this category. With a stable and wealthy patron, substantial investment and currency support came quickly and easily by comparison with the remainder, such as Ukraine, Uzbekistan or Armenia, that struggled with poverty and disinvestment. Bulgaria fell under Mafia rule, while labor unrest struck Romania. The influence of the fall of the USSR on Europe is incalculable. All the old rules of the game changed. What had once unified Europe now divided it, and old rivalries began to develop. While Russia has begin to function again on the global stage, the world will never quite recover from the end oft he cold war. The impact of child safeguarding policies How Current Policies, Theories and Politics Have Influenced Children and Young People This essay is going to look at the policies and politics about the safeguarding, welfare and wellbeing of children and young people. In particular this essay will look at the quality of social services and statistics of children’s involvement with social services, the Children’s Act 2004 and what this proposes (Kay, 1999). Also the Victoria Climbie Case and who she was involved with, Every Child Matters and the approaches taken, the Children’s Commissioner, the Children’s fund and charities such as the NSPCC, Lucy’s Faithfull Foundation and Action for Children will all be considered to understand what support is available to children and young people. In 2012/13, it is reported that just under 600,000 children had some sort of involvement with social services because of concerns for their welfare. The Department for Education (DFE) is now known to looking at the prospect of raising the quality of services delivered by social care workers. To achieve this, the DFE has sought to enforce the revised statutory guidance which promotes the welfare of children and safeguard them from harm (Morgan N and Timpson E, 2014). The result being that, child protection services not seen to be at the right level by Ofsted will be changed by the DFE with immediate effect. To improve future services the department for education are continuing to reform the system of serious case reviews to demonstrate to social workers, incidence of serious protection issues and this can be improved on. Other continuous improvement measures include the Step Up to Social Work scheme and Frontline Pilot which are now looking at attracting high-quality graduates interes ted in social work. Private and voluntary organisations are also to be delegated some of the works undertaken by social care workers in an effort to strengthen child protection services, support for looked after children and to give vulnerable children a better chance of staying with their families rather than going into care through improved early interventions services (Morgan N and Timpson E, 2014). The fundamental aim of the Children’s Act 2004 is to improve the wellbeing of children and young people through local authorities and agencies while widening the powers of relevant service providers. Safeguarding children and promotion of children’s welfare is therefore the priority of the Children’s Act. Unsurprisingly, the non-statutory Area Child Protection Committees have been replaced with Safeguarding Children Boards which means a robust plan of strategy could be set out for children and young people and could be made and published by children’s services authorities. The Children’s Act also allowed databases to be made that held all children’s and young people’s information whereby professionals involved with children or young people could access and share information (The Stationary Office, 2004). The Every Child Matters initiative (ECM) was introduced in 2004 after the sad death of Victoria Climbie who was widely known by all the relevant agencies two housing authorities, four social services departments, two child protection departments linked to the police, an NSPCC ran child unit and two different hospitals. In the case of the latter, Victoria Climbie had been seen with serious injuries (Department for Education, 2003). The ECM aim to promote the wellbeing of children and young people through better coordination between multi-agencies such as schools, local authority, police and other care providers. This approach is deemed productive as organisations can readily team up to share information in order to boost any identified child protection measure (everychildmatters.co.uk, 2014; Knowles, 2009). The establishment of Children’s Commissioner in 2005 has further strengthened the mandate of the ECM initiative as vulnerable people in society now have a voice in parliame nt and their local areas (everychildmatters.co.uk, 2014). The efforts of the government is complemented by a plethora of charity and not for profit organisations such as the NSPCC, Lucy’s Faithfull Foundation and Action for Children to name but a few. The NSPCC being the leading children’s charity for example fights to end child abuse by protecting and changing the lives of children for the better and freeing them from harmful situations. The charity will also support parents who are in need of advice in any way (nspcc.org.uk, 2015). Lucy’s Faithfull Foundation on other hand is the only UK-wide child protection charity dedicated solely to reducing the risk of children being sexually abused.
